Principal/Counselor Recommendation Form
Have your Parent/Guardian complete the top portion
- Copy of your current report card
- Copy of your most recent standardized (MEAP) test results
- $25 non-refundable processing fee payable to IA Macomb
The Principal/Counselor will complete the recommendation form and forward
it either separately or with your entire application packet to IA
Macomb.
